    Types of LASIK Surgery

    Keratoconus is a progressive disease of the cornea (The front transparent layer of the eye) characterised by thinning and buldging of the cornea.

    What is CXL?

    Keratoconus gets worse because the cornea weakens. CXL, also known as C3R, uses ultraviolet light and vitamin B2 (riboflavin) drops to stiffen the cornea. Used together, they cause collagen fibrils within the cornea to cross-link – or bond more tightly. This treatment mimics the normal age-related stiffening of the cornea, which is known as natural cross-linking. Thus it prevents further thinning of the cornea and progression of the disease.

    Which patients benefit from CXL?

    The treatment is usually recommended for patients whose corneal topography scans show that their keratoconus is getting worse. In order to obtain the most accurate scans,it is recommended that you remove your contact lenses prior to your scan (2 weeks for RGPs, 1 week soft contact lenses). If you are unable to manage without your lenses, treatment may still be available. In this situation, so long as the cornea is not too thin, treatment may be offered based on your individual estimated risk of disease progression. Age is the most important factor in determining this risk: because of natural cross-linking as you get older, keratoconususually stops getting worse by the mid-30s, so CXL is not normally required for older patients.However a regular check up with your ophthalmologist is necessary.

    What is the procedure of CXL?

    The standard CXL procedure is the “Epithelium – Off” CXL which involves removal of the superficial layer of the cornea followed by soaking the cornea with Riboflavin dye for 30 min and then exposure to UVA for 30 minutes.

    The more recent version called “Accelerated CXL” is a variation of the standard CXL which speeds up the time for delivery of UVA by increasing the intensity. The total amount of UVA delivered thus remains the same.

    After CXL your eye may be slightly red and your vision may be blurry for the first few days, it takes a few weeks for it to stabilize.

    Although a relatively safe procedure,there are certain rare risks such as – post op Haze, Infection, Sterile corneal ulceration and corneal melt.

    You would be monitored regularly and would require a contact lens trial only after 3 – 4 months following CXL.

    How much time would you need to take off work or studies?

    You should allow at least one week off while most of the surface healing occurs, or two weeks if your job involves a lot of computer work, and the treatment is being done on your better seeing eye. You will be putting eye drops in every hour for the first day, and then every four hours for the following days. Day to day activities such as watching TV or using a computer will not do any damage to your eye, but you might find it more comfortable to rest with your eyes closed early on.
    To determine if your cataract should be removed, your ophthalmologist will perform a thorough eye examination. Before surgery, your eye will be measured to determine the proper power of the intraocular lens that will be placed in your eye. Tell your doctor about all the other medications you are taking routinely and if you suffer from any other systemic condition such as diabetes, Hypertension, Heart disease, Epilepsy, Asthma etc.
